Nocturnal Ejaculation

I am suffering from nocturnal emission for many years. I visited a doctor but he said that I should continue medication for the same. This problem happens twice a week. Kindly suggest a remedy.

Nocturnal ejaculations, or wet dreams, are completely normal. The testes produce sperm throughout your life. If these are not ejaculated during sex or masturbation, they will be ejaculated at night. This is called a nocturnal ejaculation, and is normal. It does not need treatment. In fact, this is a good sign, which means your sexual system is performing properly.
Refractory Period

In your last session you had mentioned that it is not possible to increase seminal fluid. Is there a way to increase the frequency, or reoccurrence of orgasm in men? I am 54 yrs old and have one orgasm a day. My girl friend and I are together for just 2 weeks and it would be nice if I could perform more.

Most men have a "refractory period" in between orgasms, during which it takes some time for the body to go back to normal. This means that after an ejaculation, the penis becomes soft, and it takes time for the penis to become hard again, even after stimulation. In young men, this time interval can be short. As men grow older, the refractory period does get longer. There is no method to shorten this, sorry. Performance should not be equated to ejaculation - and most women are quite happy with a cuddle or being masturbated so they can get an orgasm, after you ejaculate.
Low Sperm

I am 29 yrs old. It has been one year since my marriage. My wife and I are trying for a baby but nothing seems to wok. I have gone for all tests and the problem with me is - I have a sperm count of 2million/ml and motility is 40%. Also sonography of scrotum revealed varicocele. Please advise.
Dear Amit,
I agree you have male factor infertility due to a low sperm count (oligoasthenospermia). But there is no way of improving sperm count or motility. Please get it re-checked again. A varicocele is a common condition in which the veins drain blood away from the testes and it gets swollen. This does not affect the sperm count and does not need surgery.
If your sperm count is persistently low, then your best option would be a type of test tube baby treatment called ICSI in which we fertilise your wife's eggs with your sperm in the IVF laboratory.
Failed Medication
Jennifer Esperanza asks,
I'm 23 yrs old and I have PCOS on both ovaries. My doctor had placed me on Clomid 50mg during my last cycle and I didn't ovulate. This cycle I was given Clomid 100mg a day from day 3-7 then was given Gonal 75 IU shot on day 16 however I have just taken an ultrasound on day 18 and still my largest follicle only measures 1.3 x 0.7cm. Is there a chance for me to ovulate? Can I do something to increase follicle growth?

This means you are not responding to this dose. Ask your doctor to treat you with 1500 mg Metformin daily. This will help you to ovulate. Losing weight will also help.
Pregnancy Test

My doc prescribed the following medicines when I was trying to conceive, she has asked me to take Microgest along with folic acid. Is this good? Also my HCG serum result on 27th day after last month period is 124. Does it mean I am pregnant?
Dear Deepika,
Yes, a HCG level of more than 10 mIU/ml confirms you are pregnant. You will need to repeat the level every 3 days to confirm that this is a healthy pregnancy. Microgest is progesterone. This is a natural hormone, which is used to support the pregnancy.
